To properly fill out and submit the forms, please follow these instructions:

1. Download the form to your device.
2. Open the downloaded file in Adobe Reader. If you don’t have Adobe Reader, you can download it for free from the following link: Get Adobe Reader.
3. Fill out the form and make sure to sign it using the signature field provided.
4. Click on the submit button to submit the form. Please note that the submit button will only work if you open the file in Adobe Reader.
5. If, for some reason, you cannot open the file in Adobe Reader, you can still fill out the form in your browser window. Once you have filled out the form, save the PDF with the changes.
6. Print out the form, sign it, and scan the document.
7. Attach the scanned document to an email and send it to [email protected] and [email protected].
